While your doctor determines when you are ready to be discharged, discharge planning begins almost immediately after admission and involves a case manager, social worker, and your medical team. If you are unsure about anything, don't be afraid to ask questions and take notes.

The DeTar SeniorCare Center provides health information and educational programs, medication management, laboratory and diagnostic testing, and preventive healthcare to adults age 65 and up.
The SeniorCare Center is located on the third floor of DeTar Medical Plaza West Tower (Suite 310E).
