When your doctor decides you are ready to leave the hospital, a discharge order will be written. It is important for you to make arrangements with a family member or friend to help you when it's time to go home. Some important things to remember concerning:

Personal Belongings

Collect all of your belongings and double-check closets and drawers. If you have anything stored in the hospital safe, please retrieve your belongings.

Discharge Instructions

Your doctor and your nurse will give you instructions about post-hospital care. If you have questions about your diet, activities or other matters, please be sure to ask. There are occasions when patients need additional treatment or care after they are discharged from the hospital. To meet the special needs of these patients, the DeTar Healthcare System offers some specific options:

Skilled Nursing Unit
 
The unit is designed for patients who no longer require acute care in a hospital but who need additional short-term care before discharge. Rehabilitation and specialized nursing care are emphasized.
 
The DeTar Rehabilitation Center
 
The DeTar Rehabilitation Center is staffed with licensed physical, occupational, recreational and speech therapists who treat outpatients as well as inpatients. The department specializes in the rehabilitation and therapy of orthopedic and post-amputation patients and those recovering from stroke or neurological disorders and work-related injuries.
 
DeTar's rehabilitation team offers a complete array of state-of -the-art therapeutic equipment and a skilled staff providing Physical Therapy, Occupational Therapy and Speech Language Pathology. Special services include: Aquatic Therapy with heated therapeutic pool and lift system for patient access; conventional skilled land therapies; pediatric wing with a viewing room; specialized programs for treatment from pediatrics to geriatrics, including sports injuries, pain management, orthopedic dysfunctions, neurological dysfunctions, post surgical rehab, arthritis conditions, pre-driving evaluations, birth defects, traumatic injuries/accidents, work injuries, and comprehensive patient education. The DeTar SeniorCare Center
 
The DeTar SeniorCare Center offers a wide range of comprehensive and personal healthcare services that truly make a difference to adults 65 years of age and older. In addition to your own personal physician care, we provide health information and educational programs, chemotherapy, medication management, laboratory and diagnostic testing, preventive healthcare, rehabilitation, and home healthcare. We also offer insurance billing and can help with the complicated forms and other paperwork you may need assistance with. The SeniorCare Center is located on the first floor of the DeTar Medical Plaza West Tower. For more information on the DeTar SeniorCare Center, please call 575-0228, Monday through Thursday, from 8:00 a.m. until 5:00 p.m. and Fridays, from 8:00 a.m.until 12 p.m.
